About the Role:
The Home Hospice Nursing Assistant (CNA) in Manitowoc County plays a vital role in providing compassionate, patient-centered care to individuals facing life-limiting illnesses in their homes. This position focuses on supporting patients' comfort and dignity by assisting with daily living activities, monitoring health status, and offering emotional support to both patients and their families. The CNA works closely with the interdisciplinary hospice team to ensure that care plans are effectively implemented and adjusted according to patient needs. This role requires a deep commitment to empathetic care, excellent communication skills, and the ability to work independently in a home setting.
